The system prioritizes questions you have never answered in this subtopic (since the last reset). If you request more questions than there are unseen ones, it fills the remainder with previously-seen questions at random.
Once you have seen every question, only the ones where your most recent attempt was incorrect are served. A question exits Phase 2 as soon as you answer it correctly in a new session.
